The Major Declaration Form is available in several formats:

  • Wordperfect document. In most cases, the browser will prompt users to save this file to a local disc. Once it has been saved, it can be opened and edited with Wordperfect.
  • MS Word document .
    • Some browsers (e.g., Internet Explorer) allow users to open and edit documents within the browser. Having edited the document, users can then save the file to a local machine or network.
    • Browsers may also offer the option of saving the document to a local file folder. After saving the file, it can be edited using MS Word.
  • MS Word template. This document is a "template" that can be edited to add "constant information (e.g., advisor name, e-mail address, major, major code) and saved in the "Templates" folder in the MS Word program. This may be a time-saving strategy for some users.
    • Some browsers (e.g., Internet Explorer) allow users to open and edit the template within the browser; doing so allows users to edit the document and save it to a local folder. If you wish to save the document as a document, be sure to specify a ".doc" file extension.
    • Your browser may also give you the option of saving the template. To use this document as a template, you will have to save it in the "Templates" folder within MS Word.
  • Adobe PDF file. This format can be printed out and filled in by hand. It is not a "fillable pdf" form.
