VI.33 L&S Major Declaration Form (Go to form)
The form advisors use when students declare or cancel an undergraduate major in L&S is no longer available as a three-part printed form. Instead, it is available as a form that can be downloaded from this site (see below). Advisors should download the form and follow one of the two procedures outlined below.
PROCEDURES FOR DECLARING OR CANCELLING AN L&S MAJOR, CERTIFICATE OR OPTION
For L&S Students wishing to Declare or Cancel an L&S Major or Certificate:
- Download the L&S Major/Certificate/Option Declaration/Cancellation Form (see below).
- Advisor and student fill out the Form. Please note:
- The form must be signed and dated by both the student and a representative of the Major Department.
- The form must include t he name, phone, and email address of the departmental faculty or staff advisor associated with the major. Departments may designate an individual who serves as a "general advising contact".
- For cancellation, if no other major has been declared, the advisor listed should be an L&S Advising Center or Cross-College Advising Center contact.
- Distribute copies to:
- Student
- Relevant Major Department(s)
- Registrar's Office
- The Registrar’s Office will update the student’s program in ISIS, add the advisor to new Program/Plan advisor panel, and delete old Program/Plan Advisor row if no other major is declared.
For non-L&S Students wishing to Declare or Cancel an L&S Major or Certificate:
- Download the L&S Major/Certificate/Option Declaration/Cancellation Form (see below).
- Advisor and student fill out the Form. Please note:
- The form must be signed and dated by both the student and a representative of the Major Department.
- The form must include the name, phone, and email address of the departmental faculty or staff advisor associated with the major. Departments may designate an individual who serves as a "general advising contact".
- Distribute copies to:
- Student
- Relevant Major Department(s)
- L&S Student Academic Affairs Policy Group (70 Bascom Hall)
- Student Academic Affairs (or similar) of the School/College in which the student is enrolled
- The student's School/College student academic affairs office will update the student’s program in ISIS, add the advisor to new Program/Plan advisor panel, and delete old Program/Plan Advisor row if no other major is declared.
The Major Declaration Form is available in several formats:
- Wordperfect document. In most cases, the browser will prompt users to save this file to a local disc. Once it has been saved, it can be opened and edited with Wordperfect.
- MS Word document .
- Some browsers (e.g., Internet Explorer) allow users to open and edit documents within the browser. Having edited the document, users can then save the file to a local machine or network.
- Browsers may also offer the option of saving the document to a local file folder. After saving the file, it can be edited using MS Word.
- MS Word template. This document is a "template" that can be edited to add "constant information (e.g., advisor name, e-mail address, major, major code) and saved in the "Templates" folder in the MS Word program. This may be a time-saving strategy for some users.
- Some browsers (e.g., Internet Explorer) allow users to open and edit the template within the browser; doing so allows users to edit the document and save it to a local folder. If you wish to save the document as a document, be sure to specify a ".doc" file extension.
- Your browser may also give you the option of saving the template. To use this document as a template, you will have to save it in the "Templates" folder within MS Word.
- Adobe PDF file. This format can be printed out and filled in by hand. It is not a "fillable pdf" form.
